Form not conforming to my layout

Hello,

I'm trying to put a form looking better. For that I've used the "Form editor" tool. But when I generate the from it doesn't appear as I edited it.

What can I change so it takes the shape I want?

This is what I want

But I'm getting this,

I would like to be able that all boxes end in the same column and not in the end of the text.

You need to go to the Form Editor, select the desired Group/Groups and set the Whole line Property as TRUE. See example image below.
